Credit Information Authorization

Agreement and Certification

By accepting these terms, you (including all Business Owners, Officers, and Principals listed) certify that:

• All information and documents you submit in connection with your request (“Information Request”) are accurate, true, and complete
• You will immediately notify SSBV LLC, its representative, subsidiaries, and affiliates (“BusinessLoans.com”) and any third-party lenders, finance companies, or other third parties that are involved with or provide commercial lending solutions (“Partners”) of changes to the information provided in your Information Request or your financial condition
• You understand that Partners are relying on the Information Request you provide

Credit Review Authorization

You authorize BusinessLoans.com and any Partners involved in your Information Request to:

• Conduct a complete review of you and your business to process your Information Request for business financing
• Obtain personal and business credit reports from credit bureaus including TransUnion, Experian, Equifax, and Dun & Bradstreet
• Request and receive your credit card processor statements and bank statements
• Contact banks, creditors, and other third parties to verify information provided in your Information Request
• Receive information about your commercial lease from your leasing company

Ongoing Authorization

This authorization allows BusinessLoans.com and Partners to obtain your credit reports:

• When reviewing your Information Request and/or application with a Partner
• During the closing process
• At various times during the term of any financial product for servicing or enforcement purposes

This authorization serves as your written instructions under the Fair Credit Reporting Act.

Information Sharing

You authorize BusinessLoans.com to share your Information Request and any information obtained during the review process with potential Partners. A photocopy of this authorization is acceptable for releasing credit and investigative information.

Your Authority to Act

You confirm that you are authorized to represent yourself and the business listed in your application, and to make these representations to BusinessLoans.com and Partners.

Important Notice About Accuracy

You confirm that all information provided is true and correct. You understand that:

• BusinessLoans.com and Partners will rely on this information when considering your Information Request and/or application
• Any misrepresentation may be viewed as fraud and may be punishable by law
